IOM and the Six Aims for Improvement - #6 Equitable

Care should be EQUITABLE: care that does not vary in quality because of personal characteristics such as gender, ethnicity, geographic location, and socio-economic status.

How can we provide equitable health care to geographically remote (rural) areas whereby care does not vary from that provided in more densely populated areas? Since Luft documented the maldistribution of specialty medical providers almost 30 years ago, the geographical disparity continues to effect rural populations. Despite advances in medical technology (e.g. CT now available in small hospitals, telemedicine, etc) there remain critical elements of care delivery that are subject to the physical 'hands on' presence of care providers.

Is it fair to acknowledge that care will not be equitable and yet, our goal remain to innovate to reduce essential quality variation?
